One of the questions I’ve often encountered in healthcare boardrooms is: 


"If we have a cure for Hepatitis C, why can’t we eradicate it in the U.S.?"


At first glance, it seems simple—science has delivered. But the real answer is layered, deeply rooted in public health logistics, behavior, and economics.



🔍 Understanding the Landscape


An estimated 3 million people in the U.S. currently live with Hepatitis C, though the actual number may be higher due to undiagnosed cases. Of those, almost two-thirds are either incarcerated or IV drug users—a population facing unique treatment challenges.

This matters when we think about how revolutionary drugs, like Sovaldi, are administered.



The Clinical and Financial Challenge


Sovaldi, the first approved cure for Hepatitis C, is a 12-week regimen that transformed treatment outcomes. But early on, its $100,000+ price tag made widespread rollout nearly impossible. While costs have dropped to $25,000–$50,000 with generics, the financial responsibility often falls on state programs and taxpayers, especially for incarcerated or uninsured individuals.

Even if we could treat everyone today, the risk of reinfection—particularly among active drug users—makes sustained eradication extremely difficult.
